Overview
Calcitonin is a 32-amino acid peptide hormone produced by the parafollicular cells (C cells) of the thyroid gland. It plays a role in calcium and bone metabolism, primarily by inhibiting bone resorption through direct effects on osteoclasts.
The hormone is released in response to elevated blood calcium levels and acts to lower calcium by inhibiting bone breakdown and promoting calcium excretion by the kidneys. It provides a rapid response to hypercalcemia, complementing the slower-acting parathyroid hormone system.
Therapeutically, calcitonin (especially salmon calcitonin, which is more potent than human) has been used for conditions including osteoporosis, Paget's disease of bone, and hypercalcemia of malignancy. It also has analgesic properties for bone pain.
The use of calcitonin for osteoporosis has declined with the availability of more effective treatments (bisphosphonates, denosumab, romosozumab). Concerns about potential cancer risk with long-term use have also limited its applications.
Calcitonin remains important in diagnostics, with calcitonin levels serving as a tumor marker for medullary thyroid cancer (MTC), which arises from calcitonin-producing C cells.
Discovery & History
Calcitonin was discovered independently by Harold Copp (Canada) and Paul Foster and others (UK) in 1962. Copp coined the name "calcitonin" for its calcium-lowering effects.
Initially, researchers believed calcitonin came from the parathyroid glands. The thyroid origin was established later, specifically from the parafollicular or C cells of the thyroid.
The discovery of calcitonin completed the understanding of calcium homeostasis: parathyroid hormone raises blood calcium; calcitonin lowers it. This led to therapeutic applications in conditions of excess bone resorption.
Salmon calcitonin was found to be 40-50 times more potent than human calcitonin and more resistant to degradation. This made it preferred for therapeutic use. Synthetic salmon calcitonin (calcitonin-salmon) became available in the 1970s.
Therapeutic applications developed for Paget's disease, osteoporosis, and hypercalcemia. Nasal spray formulations improved convenience for osteoporosis treatment.
In 2013, the EMA recommended restricting calcitonin use due to a small but significant increased cancer risk observed with long-term use. This, combined with availability of more effective osteoporosis treatments, has reduced calcitonin's clinical role.
Mechanism of Action
Calcitonin acts through specific G protein-coupled receptors:
**Calcitonin Receptor (CTR)**
The calcitonin receptor is expressed on target cells:
- Osteoclasts: Primary target for bone effects
- Kidney tubules: Calcium excretion
- Central nervous system: Analgesic effects
**Osteoclast Inhibition**
